psycho-social History Taking

 The term ‘psycho-social History or assessment’ is used in this guideline refers to a comprehensive assessment and history of the life of the client , as the name indicates, psycho-social is relating to the combination of psychological and social behavior .

This assessment is designed to identify those personal psychological and environmental (social) factors that might explain the background to the current symptoms. This assessment leads to a formulation from which a management plan can be developed. This history aims to undertake a thorough review of risk and protective factors,The practical aspects of conducting a psycho-social assessment are discussed in these forms which can be used as example while taking the history or assessing the client .
